NEW PATIENT FORMS

Please use the form below to request your new patient information  and then submit it.  We will promptly send you the forms via e-mail. Print them out and complete them prior to your first visit. When you arrive at our dental office, you will only need to sign for your patient information that you filled out, go over your HIPPA information, provide us a copy of your insurance card and picture ID.

FOR EMERGENCY APPOINTMENTS

The cost for the emergency exam will be $70. If an xray is needed there will be an additional charge.  This is due IN FULL at the time of the examination.  An appointment will be given to you to have the dental work completed.  A claim will be submitted to your insurance company for the examination cost and your dental insurance will reimburse you what they feel they should pay.  If you have any questions, please call and ask for Michelle as she can help you with anything.  Our telephone number is (706)855-8818.

PATIENTS THAT HAVE DENTAL INSURANCE

Our dental office participates with some dental insruance companies.  They are MetLife and United Concordia Company.  The payment that your dental insurance sends to us, is credited to your account; anything left over or not paid by your insurance company is still your responsibility.

Dental insurance companies cover between 60-100% of THEIR FEE SCHEDULE, not Dr. Comella's fee. This is a contract between you, your employer and your dental insurance company, not Dr. Comella.  If you have MetLife and United Concordia, they will cover 100% for your first time visit. All other dental insurances will have a copay. During your New Patient examination, we will also go over your dental treatment. You will be given a treatment plan showing an ESTIMATE of what your dental insurance will cover as well as your cost for each procedure.

All payments must be paid at the time of your appointment with Dr. Comella.
