Patient Information

Thank you for choosing Augusta's Smiling Dental Center as your dental provider.  As new patients, we ask that you come to the office ten (10) minutes prior to your dental appointment to fill out your new patient forms.  Please provide us with a picture ID (drivers license), your dental insurance card, and a list of your current medicaitons.
