Thank you for choosing Augusta's Smiling Dental Center/Dr. Martin Comella as your dental provider.  We are committed to you.

Please inform us immediately of any changes in your home address and telephone numbers or dental insurance information.  If you have any questions about billing, please call us at (706)855-8818 prior to your visit. 

PATIENTS WITH DENTAL INSURANCE

We will submit all dental claims directly to your insurance company for payment.  This is a courtesy to our patients.  You are expected to pay your estimated portion(s) according to each dental visit.  All co-pays, deductibles and non-covered services will be collected at the time of service.  If in the event that your dental insurance coverage changes, please notify us immediately.  We are a participating provider with United Concordia Company, Aetna, Metlife, Delta Dental and Blue Cross Blue Shield Insurance Companies.  Our office will provide you with an estimated treatment plan showing what your dental insurance estimates they will pay as well as what your estimated cost out of pocket will be.  If there is anything left over that your dental insurance does not cover, you are responsible for the balance that they do not pay.

USUAL AND CUSTOMARY RATES

Your insurance policy is a contract between you, your employer and your insurance company.  Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area.  You are responsible for payment regardless of any insurance company's arbitrary deterination of usual and customary rates.

FOR YOUR CONVENIENCE, THE FOLLOWING PAYMENT METHODS ARE ACCEPTED:

• Personal Checks
• American Express, Discover, MasterCard and Visa credit cards
• Care Credit (a financing company - 1(800) 365-8295

***there will be a fee of $35 for returned checks***

MISSED/CONFIRMED APPOINTMENTS

Unless canceled at least 48 hours in advance, our office policy is to charge for missed appointments at a rate of $50 per hour missed.  Our office staff calls to verify your dental appointment(s), 4 days to 1 week before your scheduled appointment.  An email is also sent to you.  Please return their phone call(s) or leave a message on the office answering machine.  We do need to know if you will or will not be able to come to your reserved appointment.  If you do not respond within the allotted time, we wil be forced to give your appointment day and time to another patient that is on the will call patient list and a charge for the missed appointment will be added to your account.
